The Youth Housing First program works with youth between the ages of 16-24 years to find to safe, secure and sustainable permanent housing. The Youth Housing First Team follows the Critical Time Intervention model that builds, tests, and refines connections to mainstream systems and natural supports (family and friends) to meet all of a person’s needs over a 9 month period. 

The Complex Case Coordinator will provide support to three Housing Support Workers who focus on housing and tenancy management activities. The Complex Case Coordinator completes all new intakes with youth and explains program expectations to the youth and their supports. The Complex Case Coordinator also works in conjunction with two other agency within this program and a Cross-System Support Team established to assist in the development, refinement, and communication of case plans and case management activities for youth experiencing homelessness.

Responsibilities:

  • Implement and monitor supervisory standards for three Housing Support Workers
  • Completion of administrative duties such as program forms, staff scheduling and time sheets
  • Utilize the Critical Time Intervention case management model to prevent recurrent homelessness
  • Coordinate care by liaising with  the two other agency partnerships within in the program, Cross-System Support Team, other community agencies and partners to develop a support network for the youth
  • Build positive professional relationships with related community agencies
  • Participate in community meetings, including Cross-System Support Team meetings
  • Chair Youth Housing First Team staff meetings
  • Communicate effectively with stakeholders, guardians and other professionals involved with the youth
  • Maintain information in the agency database and participate in program evaluation and statistical information

Qualifications / Skills:

  • Degree or Diploma in Human Services or related field
  • Three to five years experience working with high risk youth in the community, including outreach and residential
  • Experience supervising front line employees
  • Understanding and experience engaging in Harm Reduction, Strength Based, and Trauma Informed approaches
  • Strong knowledge of community resources for youth
  • Strong listening skills, relationship building, and assessment skills
  • Able to follow the agency’s philosophy and core values
  • Must be self-directed, flexible, team orientated and able to follow direction
  • Criminal Record Check, Child Intervention Record Check and Standard First Aid and CPR
  • A safe reliable vehicle with business insurance and a willingness to transport youth
  • All candidates will be obliged to comply with EJHS’s Mandatory Vaccine Mandate, as a condition of employment. 

Conditions of Employment:

This is an outreach position where the worker will be required to be in the community assisting the youth meeting with community partners and other youth serving agencies. There may be times where the Complex Case Coordinator will have to flex their hours to best meet the needs of the youth or support the Housing Support Workers.  Interviews will be held on February 15 & 16, 2022.

Salary: The starting salary is $52,000 annually, with a competitive benefits package available upon successful completion of the probationary period.
