Job Purpose:

Apart of Alberta’s Ten Year Plan to End Homeless, in partnership with Homeward Trust Edmonton, the Follow-Up Support Worker is a member of Boyle Street Community Service’s Housing First team.  The Housing First program is a twelve month intensive case management program targeted towards individuals and families that are chronically homeless and have multiple barriers to permanent housing.

The Housing Outreach Worker will be responsible for ensuring that chronically individuals and families referred to the Housing First Program receive appropriate assessment of their housing needs. The Housing Outreach Worker will work in partnership with landlords from the public and private sector to ensure that individuals and families find appropriate, safe and affordable housing.  

Key Areas of Responsibilities:

  • Work with community partners to connect with referrals received from Homeward Trust’s Coordinated Access 
  • Utilize the Service Prioritization Decision Assistance Tool (SPDAT) to ensure clients receive appropriate assessment of their needs and are provided with the assistance and information they need regarding services 
  • Provide short-term problem-solving, identify and assess  supports and referrals needed and inform the Follow Up Support Worker 
  • Ensure clients are fully informed of their obligations while in the Housing First Program 
  • Work in partnership with both the private and non-profit sector of the housing market to ensure appropriate placement of clients, this includes housing searches and viewings  
  • Ensure that all necessary paper work including (but not limited to) the lease agreement and move in report are completed once housing is secured 
  • Maintain a close working relationship with Alberta Works and Alberta Assured Income for the Severely Handicapped (AISH) in order to facilitate a smooth transition into permanent housing 
  • Facilitate a warm transfer meeting with the Follow Up Support Worker, Team Leader and the participant once the participant is housed 
  • Maintain accurate comprehensive documentation regarding all interactions with clients and landlords to be in accordance with agency standards, and according to funder requirements 


Education: 2 year Diploma or equivalent. Candidates without a diploma but who have post-secondary education and relevant and related experience will also be considered. 

Skills, Abilities and Attitudes:

  • Ability to work in a highly stressful and ever changing environment
  • Must have excellent boundaries
  • Mental resiliency is key
  • Great attention to detail and excellent organizational skills and ability to multi-task
  • Ability to solve problems for clients quickly, creatively and ethically
  • Capable of supervising and motivating practicum students and/or volunteers
  • Able to identify and maximize relevant opportunities


  • At least 2 years of experience in delivering community based services
  • Knowledge of Landlord and Tenant Legislation
  • Understanding of current housing market and trends, vacancy rates, and rental amounts
  • Knowledge of evidence based practices, motivational interviewing skills and cognitive behavioral techniques
  • Excellent knowledge and understanding of poverty related issues, trends and services particularly within, but not limited to the urban aboriginal population is essential
  • Valid class 5 Driver’s License and reliable vehicle for local travel on behalf of clients vehicle 
  • Criminal Record and Child Intervention Checks (results will be reviewed on a case by case basis)
