Job Purpose:

The Network Coordinator ensures the coordination of Health for Two activities in the assigned network, through building relationships with network partners, maintaining communication, promoting collaboration, and responding to needs of network partners in relation to delivery of the Health for Two Program.


  • Facilitate program delivery by network partners through promoting relationship with and amongst partners, organizing regular meetings of partners (including agenda preparation and minutes), identifying issues, problem-solving, and maintaining regular contact with partners and volunteers. Building capacity among partner agencies to support pregnant women participating in the Health for Two Program.
  • Ensure that network partners have resources, such as milk coupons, educational material, and information required for program delivery (this includes orientation of new partners or partner staff)
  • Maintain quarterly statistical information from each delivery site and gather Closed Questionnaire Forms. Ensure that the information gathered matches resources used by partner agency. Gather statistical and activity reports on client participation and network activities.
  • Promote the Health for Two program, through developing relationships with related programs and services in the network area, respond to inquiries from potential partners and related agencies, and respond to women referred to the program.
  • Contribute to regional planning and program delivery through participation in a team of Network Nurses, Support Workers, Network Coordinators, the Program Coordinator, Resource Nurse and Program Supervisor as well as other committees as appropriate
  • Assist/help develop new Health for Two Groups at different agencies and help facilitate them.


Education: Degree in Human Services field. Candidates without a degree but who have post –secondary education and relevant and related experience will be considered.

Skills, Abilities and Attitudes:

  • Knowledge of the Health for Two program
  • Knowledge of community resources and services
  • Knowledge of mental health, addictions and homelessness
  • Understanding of cultural diversity
  • Excellent verbal and written communication skills
  • Excellent organizational and time management skills
  • Capacity to work independently as well as cooperatively as member of a team

Experience and Other Requirements:

  • At least two years directly related experience working with partners to deliver a community program
  • Valid Class 5 Driver’s license and reliable vehicle
  • Valid First Aid/CPR level “C”
  • Suicide Intervention
  • Non-Violent Crisis Intervention
  • Safe Food Handling (may be provided as on-the-job training)
  • Criminal Record and Child Intervention Checks (results will be reviewed on a case by case basis)
